Please take the time to read the following information when you’re booking a consultation for the first time, or if you’re a City Natural Therapies client booking you first online consultation.

 

Key terms and conditions

  • All times/dates are in Eastern Australian time. If you’re in a different time zone – check this handy site to find a time that suits you.
  • If you’re experiencing any difficulty with the credit card payment system – please email us with the type of service, time and date you selected as well as your name and a contact number, and we’ll complete your booking.
  • Consultations cancelled with more than 48 hours’ notice can be rescheduled up to 12 weeks of the original date without a forfeit. This must be done online (not by email). Just log into your Bookeo account and go to “your bookings” to reschedule.
  • If you want to cancel a prepaid session and not reschedule at this point, please cancel your booking online at least 48 hours in advance and email us to request a refund. If the time criteria are met, you will receive a refund (via PayPal) minus a 10% admin fee – to cover the time and costs involved with using this booking service.
  • By agreeing to the terms and conditions when making your booking, you also accept the client agreement (below).
  • Gift vouchers can only be used for the appropriate service and are not redeemable for cash. Under some circumstances a voucher can be transferred in advance to another person. Please email us if you’d like to discuss this option.

 

 

Naturopathic client service agreement

Introduction

I am a naturopath in clinical practice since 1992, helping people understand and regain control of their health.


Suitability

I work with people over the age of 16. t.

All consultations are by teleconferencing (usually Zoom).

Clients most complete and return the pre-consultation form at least 2 working days before the booked time.

As not all health presentations are suitable to treat with naturopathy alone, by consenting to this agreement you agree to comply with referrals to other health professionals when directed.

 

Intention

As a naturopath my intention is to help you understand the symptoms your body is presenting, provide health education, and appropriate lifestyle and natural remedies to improve your wellbeing.

I work as a member of your health team, and am not your primary healthcare practitioner.

Please take a moment to read it carefully as it sets out your important rights and obligations and about making sure we both know where we stand. When you use my services you agree that you are over the age of 18 and willing to be bound by these Terms and Conditions. My responsibility

I provide naturopathic health education, lifestyle advice and natural remedies, in compliance with the rules and regulations of my professional association NHAA.

All supplements I supply are approved by the Australian Therapeutic Goods Administration.

If after receiving your completed pre-consultation questionnaire (at least two working days prior to your initial consultation), I consider your presentation unsuitable for my service you will be notified prior to the appointment, consultation cancelled and your fee repaid in full.

Your responsibility

By accepting this client agreement, you agree to:

  • Follow through with referrals to other health professionals when directed
  • inform me about all aspects of your health history, and keep me updated in a timely manner regarding test results, new diagnoses or treatments supplied by other members of your health team
  • commit the time needed to implement the treatment plan and be honest with me about your compliance
  • inform your other health professionals about your current naturopathic treatment, including prescribed herbs, supplements or other remedies.
  • take Practitioner only Products (nutrients and herbs) supplied me, only as directed and accept that a follow up a consultation is required within three months of a prescription being supplied, to continue taking Practitioner only Products


Problems

Any questions regarding your treatment, or if your condition changes, please contact me as soon as possible by email.

Seek medical help immediately if your health seriously deteriorates, or in the event of an adverse reaction to supplements. Stop taking supplements and seek emergency treatment with a medical practitioner or your nearest hospital. Take all remedies with you, in their original packaging.


Results

All naturopathic advice is given in good faith, based on nformation supplied by you before or during the consultation.

A timeline for possible improvements in your wellbeing may be discussed at the time of your consultation. This is a rough guideline only, and is based on you following your treatment plan in full (including all lifestyle recommendation and taking all supplements as directed) and following through with any referrals to other health professionals.

Limitation of Liability

You agree to indemnify me (hold me blameless) for any injury or illness that you might suffer or any damage or loss that might occur while we are working together.

In no event shall I be liable to you for costs, loss or damage of any kind arising out of or related to this Agreement or the services provided in relation to it. If this clause is unenforceable for any reason, my total cumulative liability for all causes of action of any kind shall not exceed the total amount you have paid me for my services.

Intellectual Property

The materials and resources that I provide to you are protected by copyright. They are for your personal use only and may not be copied or shared in any way, without my written consent.


Confidentiality

Your private information will not be shared without consent, unless required to by law.

My third-party booking website is HIPAA compliant (full details).

 

Payment Terms

All naturopathic consultations are booked and prepaid online via the booking website.

Please familiarize yourself with the 48 hour cancellation policy and rescheduling procedure (below).

All prices are in Australian dollars. Bookings are not transferable.


GST

Naturopathic consultations are GST-exempt in Australia. Practitioner only products may be subject to GST.

 

Delivery of Services

In person consultations are at the Probert Clinic (address supplied on booking), unless otherwise advised.

Distance consultations are conducted via Skype, Zoom or Facetime. If your preference changes from the one selected in your pre-consultation questionnaire, contact me asap. If there are any technological issues during the consultations, please call me on the number supplied at the time of booing to continue the consultation by phone. At other times, email is the preferred method of communication.

Cancellation policy and rescheduling procedure

You are responsible for making your own bookings online, including any changes once the appointment has been made, via the booking website.

A minimum of 48 hours is required if you wish to change your appointment, or your fee (or prepaid credit if part of a package) will be forfeited in full.

By logging into the booking website at least 48 hours before your appointment, consultations can be rescheduled up to 12 weeks from the original date, without charge.

To reschedule simply go to http://bookeo.com/gillstannard/customer and select ‘my bookings’. Click the appointment you wish to reschedule and select ‘change’, then proceed to choose a new date and time.

To cancel a session at least 48 hours in advance, follow the steps above and select cancel.  You have the option of a receiving a credit to the full value of the fee paid, to use to use for a service at a future date (within 12 months) or refund.

 

Refund policy

If you cancel online via the booking website prior to the 48 hour cancellation cut off and prefer a refund, your payment will be refunded minus a 10% admin fee to cover time and non-recoverable expenses incurred by the third-party booking portal and merchant facility. Please email me after cancelling online, to request a credit or refund. Your credit card will be manually refunded, so please allow at least a week after you have contacted me to receive your funds.

Unused or expired credits from prepaid multi-session packages cannot be refunded. However, requests for a three month extension can be made, if you contact me before the credits expire.

Consumer guarantees

Minor problems

If you have any issues with your naturopathic service or goods, let’s talk about it. Please email me promptly, so we can resolve the situation.

In event of my booking portal being temporarily offline, please email me regarding your appointment. Keep in mind the 48 hour cancellation policy, as all emails are date and time stamped.

 

Major problems

Currently naturopathy is not a regulated health profession in Australia, so disputes are not covered by the Australian Health Practitioner Regulation Agency.

I am a Fellow of the Naturopaths and Herbalists Association of Australia. If we are unable to resolve any issues regarding your naturopathic service or goods, you are entitled to contact my professional association to take the matter further.

 

Jurisdiction & Dispute Resolution

All services and goods I provide, and the above agreement, is subject to the laws of New South Wales Australia.

If you have any issue or complaint arising out of your use of my services or this Client Agreement, we agree to make a genuine effort to resolve the dispute through negotiation and discussion.

If we are unable to resolve a dispute by negotiation and discussion within 14 days, the parties must proceed to mediation with the assistance of an accredited mediator who is independent of the parties.  The mediator is to be appointed by agreement of the parties or, failing agreement within twenty-one (21) days of the first notification of the dispute, by a person appointed by the Chair of Resolution Institute, (ACN 008 651 232, Level 2, 13-15 Bridge Street, Sydney NSW 2000; telephone: (02)92513366, email: infoaus@resolution.institute) or the Chair’s designated representative. The Resolution Institute Mediation Rules shall apply to the mediation. We agree to share the costs of mediation equally between us.

Litigation is to be considered a last resort and may not be commenced until, in the opinion of the independent mediator, the potential for negotiation and mediation have been exhausted

 

Mentoring Client Service Agreement

Introduction

I am a naturopath and mentor with more than 30 years in practice, supporting health professionals and entrepreneurs to grow a business they love.

I established a successful multidisciplinary clinic in the Melbourne CBD on graduating in the early 1990s, running it for 23 years before relocating to Sydney.

As one of the first naturopaths in Australia to formally offer both business mentoring and professional supervision, while remaining in clinical practice, I have comprehensive business and clinical knowledge and have thrived in an ever-changing market. I am an earlier adopter of online business and experienced in many aspects of the small business management – from home-based to a commercial, multi-practitioner practice.


Suitability

I work with health practitioners and small business owners, who want a career in alignment with their values, and pursue an authentic and fulfilling way of working.

Naturopaths, nutritionists and herbalists can also access professional supervision, to improve prescribing and case management.

I work with emerging, mid-career and late career practitioners and entrepreneurs.

Intention

My approach is heart-centred, individualised and practical, to help you create or reshape YOUR business, not a carbon copy of someone else’s in your field.

Exploratory mentoring sessions focus on where you are currently at in your business, strengths, weaknesses, dreams and aims. If time permits, it may include problem identification and steps to solving them. After the session, you will receive a written report and steps you can initially take to grow or redefine your business.

Ongoing or one off sessions (after the initial exploratory) focus on meeting your goals. This may include practical support in growing or reshaping your business, improving client attraction and retention, authentic ways of promoting your enterprise, improving business (and clinical) competence, confidence, holistic business skills and self-care.

I have created unique processes such as skills audits, procedures and clarifying boundaries.

I call upon >30 years as a business owner, practitioner, teacher and facilitator, to provide relevant and up to date support for health practitioners and entrepreneurs.

I am one of only a select few practitioners, officially sanctioned by the NHAA as a mentor and supervisor.

Peers and industry leaders consider me to be an expert in many aspects of small business as well as clinical experience. I have been invited to speak at numerous industry conferences, and contracted by at least two large health companies to deliver ongoing business and practice management education.

The NHAA acknowledged my distinguished contribution to the profession, by making me a Fellow of the association in 2015.

I commit to holding the space for you to pursue a way of working that best suits your desires and ability, and delivering the best advice based on my extensive experience as a clinician and small business owner.

Your responsibility

By accepting this client agreement, you agree to:

  • Commit at least two hours a week to work on your business and follow through with suggestions made in mentoring sessions.
  • Complete and return the pre-session questions at least two working days before the initial mentoring session.
  • Be honest, communicate your situation/needs clearly and be willing to commit to grow/reshape your business.
  • Turn up to sessions on time.
  • Manage your own bookings online and honour the 48 hour cancellation policy (details below).
  • Accept that mentoring suggestions do not constitute medical, financial or legal advice, so do your own research and seek appropriate professional advice before putting my suggestions into practice.

Any problems with the online booking website, or if you need clarification regarding what we have discussed in a session, please contact me as soon as possible by email.


Results
